At 4 different locations throughout Precinct 2, we will deploy 2 ACCESS2HEALTH Pods (for a grand total of 8 Pods) which we anticipate will lead to success in decreasing health disparities, increasing access to care and improving health outcomes within these communities. Transportation to and from the Medical Center is a barrier for many families who reside in the four communities we have targeted. We alleviate this barrier by bringing healthcare access to their immediate surroundings and encourage families to seek preventative and routine healthcare. The services offered within these Pods are Primary Care Services such as diagnosis and treatment of common acute and chronic disease, laboratory tests, and preventative health screenings. Mental Health Services such as individual and group counseling, treatment of depression, PTSD, substance abuse and other mental health needs are addressed. By promoting a culture of health that engages, empowers, and educates Precinct 2 residents by improving access to care, providing comprehensive services and resources we can offer opportunities for a better quality of life. Brief Description of Program Goals: • Deploy 8 Modular Healthcare Pods (2 per community in 4 communities) • Target 4 vulnerable communities for deployment of Pods (JFA, Mangum-Howell, Pep Mueller, Cloverleaf Park) • Provide Primary Care Services • Provide Mental Health Services • Target uninsured, underinsured, and low-income households • Promote culture of health and decrease health disparities through access to preventative care and routine examinations to improve health outcomes
