Purpose: The South Dakota Department of Health seeks to improve pregnancy outcomes, child health and development and early childhood systems by implementing Nurse Family Partnership and other evidence-based home visiting models to be determined in identified at-risk counties to be determined. New models will be chosen based on evidence, usability, and supports available. Communities will be identified based on capacity, fit of early childhood home visiting with current initiatives, and readiness for such services.
Goals and Objectives:
Goal 1: Identify and build relationships with community leaders, tribal entities, and partner agencies who may collaborate to implement home visiting services in existing or new communities by September 29, 2024
Objective 1: Use implementation science principles to evaluate community capacity and home visiting model fit to identify how the SDDOH will implement MIECHV in existing and new communities.
Objective 2: Identify which communities and models will require implementation via subrecipient partners and use the SDDOH Request for Proposals process to identify such partners.
Objective 3: Investigate evidence based and promising approach home visiting models for implementation in communities and develop partnerships with model developers on implementation and training if new models are identified, keeping in mind individual community needs and culture
Goal 2: Support the implementation of the expanded statewide home visiting program that meets program, subrecipient, and federal grant requirements through September 29, 2024
Objective 1: Develop and implement an onboarding plan for new Home Visiting program-level staff
Objective 2: Support expansion and realignment of current nurse-based service teams and structure
Objective 3: Develop and implement an onboarding plan for new Home Visiting sites
Objective 4: Provide Performance Reporting and Continuous Quality Improvement support to home visiting teams per MIECHV grant requirements
Goal 3: Advance Early Childhood Comprehensive System Development in identified at-risk communities by September 29, 2024
Objective 1: Continue as an active partner in the SD ECCS grant team, led by the Center for the Prevention of Child Maltreatment
Objective 2: Identify existing or develop new local Community Advisory Boards in communities identified for service delivery to ensure that diverse perspectives are represented in discussions of fit and feasibility of maternal and early childhood home visiting services.
Methodology:
• Communities served will be those identified in the 2020 Statewide Home Visiting Needs Assessment (Revised), after assessing number of eligible families and community capacity to support home visiting.
• The South Dakota Home Visiting Program will complete a Request for Proposals process to identify potential subrecipients for service delivery and models(s) to implement as the Program currently implements services only with SDDOH employees and the Nurse Family Partnership model.
• The current caseload of family slots for FY2020 is 116, and for the FY22 project period will be increased to 155. However, final projected caseload of family slots is to be determined pending identification of communities to serve and model(s) to implement.
• The DOH works with state agency partners, private service providers, and tribal entities to coordinate services for young families in the state. SD Home Visiting is and active partner in the state’s ECCS. Home visiting staff network at the local level to maintain a referral base, program growth, and coordination.
