Purpose: Louisiana has consistently ranked poorly in comparison to the rest of the country on overall maternal and child health outcomes and social determinants of health. The LA MIECHV Formula grant project proposes to strengthen, enhance, and sustain effective, evidence-based home visiting services in
at-risk communities across the state, as identified by the most recent needs assessment. The activities used to achieve the goals and objectives of this project include developing and retaining the MIECHV infrastructure and direct staff positions in order to continue to implement and provide high-quality home visiting services with fidelity to the Nurse-Family Partnership (NFP) and Parents as Teachers (PAT) models, and to advance the integration of LA MIECHV within Louisiana’s early childhood system of care.
Goal(s) And Objectives: Goal 1: Sustain evidence-based home visiting and improve maternal, child and family health and well-being. Objectives 1.1 to 1.2: To sustain the implementation of NFP and PAT. Goal 2: Provide infrastructure to support the effective implementation and expansion of evidence-based home visiting. Objectives 2.1-2.6: To provide infrastructure to support the regular provision of professional development, dedicated data capacity, program development activities, and administrative support via dedicated support from the Clinical Developer, Data Analyst and Data and Quality Manager, MIECHV Coordinator, and Statewide Administrative Assistant, respectively. Goal 3: Strengthen the implementation of MIECHV with a continued focus on the enrollment and retention of families in evidence-based home visiting services. Objective 3.1-3.2: To strengthen implementation via continued supportive efforts of the Outreach Manager and implementation of a central intake system. Goal 4:
Promote and fully integrate MIECHV within Louisiana’s comprehensive, early childhood system of care. Objectives 4.1-4.4: To support integration via continued participation in key workgroups, dedicated support from the MIECHV Coordinator and Outreach Manager, cross-system training opportunities, and collaboration with local early childhood leaders. Goal 5: Sustainable funding for evidence-based home visiting programs. Objective 5.1-5.2: To sustain MIECHV services and supports via the continued development of a sustainability plan with emphasis on deepening community investment and diversifying funding sources.
Methodology: Evidence-based models: NFP and PAT.
Communities served and Target population group(s): 39 parishes identified as at-risk in 2020 Needs Assessment and 2021 Early Childhood Risk and Reach report. LA MIECHV prioritizes serving low- income families in these communities. To be eligible for NFP or PAT services through LA MIECHV, families must also be eligible for Medicaid, SNAP, WIC, TANF, or SSI at enrollment. Additional subpopulations are represented in LA MIECHV’s client base, but current eligibility criteria does not specifically select for them.
Proposed caseload of MIECHV family slots: FY 22: 1,744; FY 23: 1,744.
Current caseload of MIECHV family slots: 1,767.
Appropriate linkages and referral networks to other community resources and supports are ensured via the collective work of statewide home visiting staff and state-level leadership. Key activities include continued support of the MIECHV Outreach Manager to provide oversight a strategic direction for ten regional Outreach Specialists, continued progress toward statewide implementation of a central intake system, and continued co-leadership of the Community Advisory and Action Teams in each of the nine LDH Administrative Regions.
