Fortune Society's Continuity in Treatment program will engage 250 incarcerated adults within 4 months of release in discharge planning and enroll 200 released adults with a substance use disorder (SUD) or co-occurring disorder (COD) into treatment and other reentry services. Evidence-based interventions will be used, including Moral Reconation Therapy (MRT), Motivational Interviewing (MI) & Seeking Safety. The populations of focus are male and female inmates being released from NYS Department of Correction & Community Supervision Correctional Facilities and NYC Department of Corrections Correctional Facilities. The program will enroll 70 clients in Year 1and 2, and 60 in Year 3. Project goals include: transitioning individuals in need of SUD treatment from incarceration; stabilizing participants in recovery from SUD/COD; assisting participants to become fully functioning parents, employees and citizens; and strengthening partnerships and systems of care for the reentry population. Pre-release services will include: outreach, screening, and transitional planning. Post-release services will include: coordination with community corrections, group MRT and twice monthly individual MI sessions, and referrals to other services, including mental health, health care, education, job training, housing, and family services. Key outcomes include: clients enrolled will successfully complete MRT, Seeking Safety, MI; 65% of clients will achieve at least 90 days sobriety; 60% will report no or reduced use of alcohol or illegal substances over the past 30 days at 6 month follow up; all clients needing such services will be referred to health services, including HIV and Hepatitis testing; all clients who test positive for HIV or Hepatitis will be referred to medical services; all clients in need of mental health services will be referred to treatment; all clients for whom such referral is appropriate will be referred to education and/or employment services and family services.
