Funding preference: Health Professional Shortage Area (HPSA) Description: Margaretville Hospital (MH), a member of the Westchester Medical Center Health Network (WMCHealth), operates a 15-bed rural Critical Access Hospital in Margaretville, NY that serves northeastern Delaware County in New York State’s Mid-Hudson Region. MH proposes the Hudson Valley Strategic Training and Response Outreach for Key Emergency (STROKE) Care Network to improve care for stroke patients across rural communities in New York’s Hudson Valley Region. The proposed network will facilitate enhanced educational opportunities for EMS providers and other community members, enabling improved recognition and response during stroke emergencies. Additionally, STROKE Care Network members will work to coordinate care in a manner that expedites treatment for high acuity patients in the most appropriate care setting. The target population served by this project will be individuals age 65 or older residing in the rural counties of Delaware and Sullivan, and the rural portion of Ulster County in New York’s Mid-Hudson Region. To ensure this population is adequately served by this proposal, MH has secured commitments from community providers throughout the region to serve as Network Partners for this proposal, including MH, Mountainside Residential Care Center, Delaware County EMS, Sullivan County EMS, Ulster County EMS, the Delaware County Office for the Aging, and HealthAlliance Hospital. In addition to these Network partners, MH will leverage resources and input available from the Society of Vascular and Interventional Neurology (SVIN) and New York State Bureau of Emergency Medical Services to achieve the STROKE Care Network’s proposed focus area of developing an actionable plan to enhance care coordination and capacity among rural service providers as a means to improve health outcomes for patients who experience a stroke.
