(HCCH) is a 501c3 health center serving the needs of homeless and low-income housed but uninsured or under-insured residents of Orange, Osceola and Seminole Counties in Central Florida. The mission of HCCH is “to provide quality health care services that improve the lives of the homeless and medically indigent people of our community." HCCH, d/b/a Orange Blossom Family Health (OBFH), takes a multi-disciplinary approach to providing quality, compassionate and culturally appropriate health care services, integrating medical, dental and behavioral health services. In 2021, HCCH cared for over19,000 unduplicated patients through multiple health delivery sites and mobile health units. Over 90% had income at or below 100% of the Federal Poverty Level. HCCH is a Federally Qualified Health Center and designated as a Level 3 "Patient Centered Medical Home" by the National Committee for Quality Assurance (NCQA). Need to be Addressed: In 2020, there were 592 reported new HIV diagnoses in the Orlando Metropolitan area, which includes Orange, Osceola and Seminole Counties, the service area for HCCH. Data provided within the 2019 HIV Surveillance Report from the Centers for Disease Control and Prevention (CDC) indicated the Orlando Metropolitan area in Central Florida ranked number five in the country for new HIV diagnoses and number two in the State of Florida. Osceola County is home to 1,592 persons with HIV (PWH) and is a designated dental provider shortage area. PWH may face increased oral health concerns resulting from their HIV status and potentially from the medications prescribed to achieve viral suppression. Proposed Project and Services: Under the Infrastructure Development option, sub-category of Office Dental Equipment for Expanding Dental Service Capacity, HCCH will acquire dental equipment and supplies to create a new oral health care access point at Orange Blossom Family Health (OBFH) in Kissimmee, Florida (Osceola County). The expansion of the existing dental program at HCCH will place consistent oral health services closer to PWH in Osceola County and surrounding areas. Population to Be Served: HCCH will provide enhanced access to oral health services for PWH in Osceola County and surrounding areas who are low-income, underserved, uninsured or under-insured, and/or those who qualify under Ryan White programs.
