The Department of Health and Human Services’ (DHHS) vision is to “advocate for, support, and serve all individuals and communities in Utah. We will ensure all Utahns have fair and equitable opportunities to live safe and healthy lives. We will achieve this through effective policy and a seamless system of services and programs.” Impacting the social determinants of health (SDoH) is an evidence-based approach to DHHS’ vision of ensuring all Utahns have fair and equitable opportunities to live safe and healthy lives.
Social determinants of health are the conditions in the environments where people are born, live, learn, work, play, worship, and age and have a major impact on people’s health, well-being, and quality of life. They also contribute to health disparities and inequities. They can be grouped into five domains that include 1) Economic stability, 2) Education access and quality, 3) Health care access and quality, 4) Neighborhood and built environment, and 5) Social and community context.1 Public health action, along with multi-sector partnerships, are necessary to improve the conditions in people’s environments to ultimately promote attaining the full potential for health and well-being for all.
To impact the social determinants of health at the community level, there must be sufficient resources invested in the community to drive equitable access to health, education, transportation, housing, food security, income, etc. South Salt Lake is a geographically defined community with a brief history of investing resources into its community to positively impact the social determinants of health through policy, systems and environmental changes. Promise South Salt Lake has created the infrastructure to provide alignment and leadership to address SDoH at the individual, community, and systems level.
Promise South Salt Lake, in partnership with the United Way of Salt Lake and the Utah Department of Health and Human Services, will develop a multisector action plan to address SDoH. The purpose is to accelerate action in South Salt Lake that will lead to improved chronic disease outcomes among persons experiencing health disparities and inequities. Collaborative planning among public health, government, business, non-profit organizations, and healthcare partners is essential for addressing SDoH. We will collaborate with PSSL to implement the two objectives of this funding opportunity: 1) Convene and coordinate a leadership team of multisectoral partners, and 2) Develop an implementation-ready SDoH accelerator plan to reduce chronic disease outcome disparities in the South Salt Lake community.
