Using Systems Change Models to Promote CVD Risk Reduction and Expand CVD Screening and Treatment Services Among Vulnerable Female Populations in Delaware NOFO: CDC-RFA-DP18-1816 Project Abstract Summary Cardiovascular Disease (CVD), including heart disease, stroke, and other vascular diseases, is a major cause of morbidity and mortality in Delaware. In 2015, heart disease and stroke accounted for 22.6% and 5.4%, respectively, of all deaths statewide. To optimize CVD outcomes among vulnerable Delawarean women, the Screening for Life (SFL) program will collaborate with internal and external partners to extend cardiovascular health services to SLF-eligible low-income, uninsured, and underinsured Delawarean women age 40-64. Service expansion includes CVD risk assessment by SFL providers, personalized risk reduction counseling, medical follow-up, and referrals to community-based resources. The proposed Core and Innovation Component Models (a) aim to replace the current disease treatment CVD context with one emphasizing CVD prevention and wellness; (b) improve quality of CVD care among the target population; (c) guide vulnerable Delawareans toward making informed choices; (d) strengthen community-clinical partnerships; and (e) encourage health policy action to continue reducing the statewide CVD burden beyond the 5-year award period. While complimentary in nature, the Core and Innovation Models address distinct aspects of Delaware’s current CVD care model. The Core Component Model outlines three primary systems change strategies (CS1-CS3) to optimize cardiovascular health within the target population: • CS1: Track and monitor clinical measures shown to improve healthcare quality and identify patients with HTN • CS2: Implement team-based care to reduce CVD risk with a focus on HTN control and management • CS3: Link community resources and clinical services that support bi-directional referrals, self-management, and lifestyle change for women at-risk for CVD The Innovation Component Model also employs three main strategies (IS1-IS3) to reduce CVD burden among the target population: • IS1: Identify and target hard-to-reach and medically-underserved (HTR/MU) women • IS2: Implement novel strategies designed to enhance referral, participation, and adherence in cardiac rehabilitation programs in traditional and community settings, including home-based settings • IS3: Implement novel approaches to facilitate bi-directional referral between community programs/resources and health care systems Each Core and Innovation Component strategy is linked to measurable activities and anticipated outcomes. Year 1 goals focus on building infrastructure and capability while expanding cardiovascular services at up to ten percent of SFL provider sites. Critical evaluation of Year 1 activities will guide and cardiovascular service expansion among additional SFL provider sites during Years 2-5 of the grant award period.
