HIV/AIDS remains a major public health setback in Ethiopia. Ethiopia has a mixed-HIV epidemic with national prevalence of 1.18%. Currently, these priority populations in Ethiopia include female sex workers, individuals who engage in sexual activity with FSWs, long distance truck drivers, uniformed services, never-married sexually active females, discordant couples, migrant laborers, and adolescent and young girls. In 2014, UNAIDS launched a 90-90-90 strategy to achieve a sustained epidemic control by 2020 and end the epidemic by 2030. Ethiopia is working to achieve these ambitious 90-90-90 targets by 2020 and targeting interventions on its key and propriety populations. Currently, there are many types of organizations working on HIV prevention activities throughout Ethiopia. The specific response targeting Ethiopia?s key and priority populations is not well coordinated. Poor coordination and standardization of activities is resulting in inefficiencies and redundancies throughout the system. At the service delivery level this results in disparate and inequitable service provision. This is further compromised when existing standards are not updated and improved based on program data. There is also a lack of sufficient and reliable epidemiological data both at national and sub-national levels for HIV testing services, which is the entry point for the successful implementation of the 90-90-90 strategy. HIV/AIDS Programs are coordinated through and led by the Federal HIV/AIDS Prevention and Control Office (FHAPCO) and the national AIDS Council, and involve a range of institutions, including but not limited to the Ministry of Health (line ministry), the Ethiopian Public Health Institute (EPHI), the pharmaceutical Fund and Supply Agency (PFSA), and Food, Medicine and Heath Care Administration and Control Authority (FMHACA). Within Ethiopia?s Federal system, there are also regional HAPCO?s, regional health bureaus (RHBs), and Regional AIDS Councils whic h are responsible for the HIV/AIDS response at regional level. At regional level the HIV/AIDS multi-sectoral response have two types of offices or coordination arrangements: the first one is independent HIV/AIDS Prevention and Control Office (HAPCO) structure and HIV/AIDS multi-sectoral response coordination within the Health Bureaus lead by deputy head of bureau or a department within the bureau. The HAPCO coordination role for HIV prevention and treatment and care& support programs starting from federal to grass root structure level kebeles by CBO and FBOs. Concerning monitoring and evaluating system of the HIV/AIDS national response, FHAPCO developed an M&E framework and costed plan in line with the new strategic plan and the office is tasked with the role of developing/strengthening a system for tracking the multi-sectoral response exterior to the health facilities at all levels. It is also responsible for conducting regular joint integrated supportive supervisions and joint review meetings involving all major stakeholders. Reports at regional and national level are also coordinated by the office. Programmatic evaluations are also under the mandate of this office. As a result of the multi-sectoral response in the country, nearly 2 million MARPs & Vulnerable population are reached with targeted Behavioral Change Communication programs on yearly basis for the three years, around 9 million people are tested each year for the last five to six years, PMTCT coverage also increased from reached 58% and cumulative PLHIV who are currently on anti-retro-viral treatment (ART) 426,472 in June 2017. In terms of achieving the three 90?s target the country reached 72%, 66% and 64% for the first, second and third 90?s respectively as of June 2017.
