Many gaps exist in the continuum of HIV services for key populations (KP) and People Living with HIV/AIDS (PLWH) in the Kyrgyz Republic (KR). HIV prevalence among PWID is high – 12.4% in KG. However, HIV testing among PWID remains low, in spite of the fact that HIV epidemic is driven by injecting drug use. Quality-assured rapid HIV testing does not exist in KR. In KG, only 16.7% of ART-eligible PLWH are on ART. Furthermore, the quality of monitoring of treatment needs to be improved through and viral load testing. There are only two laboratories in KG, where viral load testing can be performed. In spite of decentralization of services for PLHIV is happened, there were no mechanisms and logistics considered to support this process. The KR healthcare system inherited from the Former Soviet Union a model where TB, STI, Narcology, and AIDS services were provided by separate vertically organized Centers until recently. The lack of integration of HIV services with other health services that are utilized by KPs creates significant obstacles for these patients’ treatment and care. Moreover, Non-Governmental Organizations (NGOs) are also limited in their capacity to coordinate their services (such as social support services and peer to peer consultations) with medical facilities to improve patient referral to services, retention in care, and adherence to treatment. TB is still a leading cause of mortality among PLWH, and management of other opportunistic infections also needs to be improved. The capacity of medical staff and decision makers to produce reliable data through evaluations and routine monitoring and to use the data for decision-making needs to be improved. The application to this FOA aims to increase access to HIV services among PLWH and PWID through improving the continuum of care and addressing the related issues. The overriding goal of HIV national program complies with PEPFAR goal in Central Asia is to reduce new infections and to provide adequate treatment and care services to those infected with HIV according to international standards. The objectives of the FOA will be to: 1) Implement a national policy for HIV RT and system certification of HIV RT providers in KR by utilizing international experts with international experience; 2) Assure high quality of HIV care, treatment, and viral load services, including ART, opportunistic infections (OI) diagnosis, treatment and prevention with focus on tuberculosis (TB) and sexually transmitted infections (STIs); 3) Provide high quality comprehensive HIV prevention to KPs; 4)Increase availability and use of quality data through routine monitoring and evaluation, and surveillance to support quality of HIV program planning and implementation. The key expected outcomes follow the continuum of HIV services by the end of the project period are: increased knowledge and skills in clinical staff on HIV care and treatment in quality assured manner; increased number of certified clinicians; increased capacity of RAC staff to coordinate prevention work in HIV program; enhanced capacity of medical staff to coordinate prevention work in HIV program; increased capacity of medical staff to use, analyze and interpret EHCMS/LIMS data; improvement in HIV services within entire continuum of care; improvement in HIV testing procedures and policies at the National HIV Program in compliance with international standards and accredited; increased number of sites using HIV RT with assured quality; increased number of PLHIV who receive HIV care and treatment services in a quality-assured manner; increased number of PLHIV who receive quality-assured prevention services; improved HIV program data quality and efficient use of data for planning and program implementation; and as a long term outcome we anticipate increased capacity of RAC staff in implementation and monitoring of HIV National Program; improved quality of HIV testing, including RT; increased coverage of HTC among PWID and other KPs
