Project Name: Los Angeles HIV and Substance Use Disorder Integration Project (LA HIP) Project. The primary purpose of the project is to provide persons with Co-occurring disorders (COD) with substance abuse (SA) treatment and who are at risk for HIV with HIV prevention/rapid testing, Hepatitis (B/C) testing/treatment, outreach, case management, and linkage to mental health, primary care, and wrap-around services. This will primarily be achieved at the project’s hub – JWCH (an FQHC) Behavioral Health and Medical Site on Vermont Avenue (Vermont Site) and the Center for Community Health (CCH). Population to be served:The geographic area to be served is the Service Planning Area (SPA) 4, which includes East Hollywood and Skid Row section of Downtown Los Angeles. The target population is homeless men who have sex with men (MSM) of age 18 and above, in SPA 4 (between Skid Row and Hollywood). The majority of the homeless population in Skid Row is affected by chronic substance abuse and/or mental illness. East Hollywood has the second highest gap in services for homeless persons with COD who are at risk for HIV. Forty-one percent (41%) of the homeless population in LA County have experienced or been diagnosed with alcohol/drug problems (Greater LA Homeless Count, 2009). Also, emerging from the 2013 Homeless Count by the LA Homeless Services Authority (LAHSA) are the following characteristics of LA homeless population: adult men represent 60% and adult women represent 32% of the homeless in LA, and twenty four percent (24%) of chronically homeless people have been diagnosed with a mental illness in the past 12 months. LAC represents the second most impacted metro city in the U.S. with about 60,000 persons living with HIV/AIDS. Skid Row (located in SPA 4) has among the highest HIV burden in LAC, the second highest. Program interventions/services: In the first year, 56 clients will receive Matrix Model (MMT) SA treatment; 68 in subsequent years. Treatment will be reinforced by intensive case management. In year one 300 clients will receive rapid HIV and Hepatitis C/B testing; 400 in subsequent years. A minimum of 60% of clients in treatment will be linked to HIV prevention, PrEP/PEP, mental health, and supportive housing services. Also, 4 persons successfully completing the SA treatment program will be trained as Peer Educators (Peers) using the Community PROMISE model. The total number of homeless individuals receiving services over the life of the grant period will be 2,887; of this number 260 will receive SA treatment services to address COD.
