The Federated States of Micronesia?s network of state-based family planning clinics and Community Health Centers, serving publicly-funded family planning clients, along with a few private providers, are the foundation for providing high quality reproductive health services to the nation?s population, particularly women of child-bearing age. The FSM continues to experience high total fertility rate (3.13 children per woman); high adolescent fertility rate (24.4 per 1,000 females aged 15-17 years); high perinatal mortality rate (26.6 per 1,000 live births); high percentage of low birth weights (6.5%); and low most or moderately effective contraceptive prevalence rate (58%). The consistently low coverage of most or moderately effective contraception implies a need to strengthen the way family planning program and related preventive health services are administered. Further, it implies that the hard to reach communities and vulnerable populations, particularly adolescents, continue to have limited accessibility to acceptable and effective family planning methods and ongoing high quality family planning and related preventive health services. Finally, these figures also imply a need to improve efficiency and effectiveness of the family planning program and related preventive services.The Family Planning Title X program will continue to address issues relating to those women at risk by virtue of their age, parity, spacing, marital status and/or physical health. However, high priority to be particularly addressed during this grant period are the vulnerable population of teenagers or adolescents and the hard to reach populations in the highlands of the main islands and on the outer islands. States will ensure services are provided in private and confidential settings and are well integrated with other reproductive health services, including obstetrics care, HIV/STI counseling and testing, breast and cervical cancer screening, substance abuse disorder s creening and with linkages for treatment of sexual violence. In addition, the Family Planning program will be strengthened through improving quality through policy and procedure updates, electronic documentation of program services including the FPAR, capacity building, and reproductive health planning. Much attention will be focused on promoting most or moderately effective methods through patient-centered, non-coercive education and counseling.Throughout this funding cycle the FSM Family Planning Program will be guided by four overarching goals that align with the Title X 2019 program priorities and key issues: 1) Improve family planning service quality; 2) Increase accessibility to a broad range of methods through voluntary, client centered Family Planning services;3) Increase access to care for Family Planning services to vulnerable and hard to reach populations; and4) Improve pregnancy planning and spacing and prevent unintended pregnancy.
