Our project consists of the expanding our Englewood building to the north to establish interior build-out of our first floor and partial buildout of the second floor consisting of 7,000 square feet. The expansion will provide for a masonry construction for a Dental Clinic, 5 additional exam rooms, nutrition/wellness program, and behavioral health services. The Dental Clinic will consist of 1,800 square feet containing 3 operatories. The Dental facility will be designed to allow entrance from the clinic and from the street so that operations can occur independently of the health clinic next door. Five additional exam rooms will be added to reduce patient wait-time due to growth, and to allow us to integrate primary healthcare services, behavioral health, and dental services using a multidisciplinary team approach to more fully address the spectrum of problems that our patients bring to their primary care provider. Phase -1 expansion will also include a partial build-out of a Nutrition/Wellness Center to incorporate patient education programs; encouraging exercise and nutrition as vital to a healthy lifestyle, and to meet the growing patient demand for more preventive health services, along with two program offices for more outreach activities. For Phase 2, (and upon identifying additional funding) this will allow us to complete the expansion of the second-floor build-out to include: cubicles spaces for more care coordination and outreach activities, one waiting area, storage room, Board of Directors room, and restrooms. In addition, this expansion will allow us to operate with sufficient space, reduce patient wait time and increase accessibility for health care, dental care, behavioral health, and nutritional/wellness activities. This expansion will allow us to increase access for 3,108 new patients (a 31% increase in new patients) which will generate approximately 9,324 additional visits, bringing our annual visits to 54,176. A 21% increase in visits at our Englewood site, and a 31% increase in revenue.
