The South Dakota Department of Health (SD DOH) is applying for funding for, CDC-RFA-DP18-1815PPHF18--Improving the Health of Americans Through Prevention and Management of Diabetes and Heart Disease and Stroke. The purpose of the project includes implementing cross-cutting public health strategies to address chronic disease prevention focused on diabetes and heart disease and stroke. South Dakota’s approach to the project involves working synergistically across programs, and with internal and external partners in order to leverage resources and relationships that will produce the greatest impact and reach for diabetes prevention and management and heart disease and stroke prevention. SD DOH will build on the collaborative work achieved previously, expand the reach of diabetes prevention and management programs, and establish systems-level change for prediabetes screening and referrals. SD DOH will also work collaboratively across programs to increase use of Medication Therapy Management, continue to develop Community Health Worker infrastructure, and promote quality improvement initiatives and team-based care. Lastly, SD DOH will facilitate use of self-measured blood pressure monitoring and enhance screening and referral systems around hypertension, high blood cholesterol and stroke. The SD DOH will maximize partnerships with healthcare systems, tribal organizations, quality improvement organizations, and academic institutions to implement evidence and practice-based interventions that will achieve long-term outcomes to improve diabetes prevention and control and heart disease and stroke prevention and control. Strategies focused on diabetes prevention and management will collectively address systems-level change for screening and referral systems, increased health plan coverage of the National Diabetes Prevention Program, as well as address program availability and reach and engagement of pharmacists across SD. Strategies to address heart disease and stroke prevention and control will address adoption of evidence-based quality measurement, engagement of non-physician team members, promote self-measured blood pressure monitoring programs, and implementation and enhancement of systems around referral for hypertension and high blood cholesterol. The Diabetes Prevention and Control and Heart Disease and Stroke Prevention Programs will work collaboratively on increasing use of Medication Therapy Management. The Heart Disease and Stroke Prevention Program and Diabetes Prevention and Control Programs will also collectively address statewide infrastructure and sustainability for Community Health Workers. The strong partnership with the South Dakota State University College of Pharmacy, Great Plains Tribal Chairmen’s Health Board, American Heart Association, HealthPOINT, Great Plains Quality Innovations Network, and Sioux Falls Health Department will provide a broad statewide base to reach high burden populations through utilization of evidence-based practices and training.
