All Idaho TB cooperative agreement-funded activities are coordinated by the Epidemiology Section Manager in the Bureau of Communicable Disease Prevention (BCDP) at the Idaho Department of Health and Welfare (IDHW), Division of Public Health (DPH). Prevention and control activities are guided by the TB Controller and a Public Health Physician, both of whom work in the DPH. The BCPD Epidemiology Section Manager is responsible for drafting and monitoring subgrants with Idaho’s 7 public health districts (PHD) to carry out essential TB prevention and control activities. Human Resource Development activities are developed and coordinated by the Epidemiology Section Manager, TB Controller, and Public Health Physician. Laboratory activities are performed by the Idaho Bureau of Laboratories (IBL). As outlined in the Project Narrative section of this application, during the project period, the Epidemiology Section Manager, the TB Controller, and the Public Health Physician, hereinafter referred to as “TB program staff”, intend to build on case detection and management activities to improve and streamline these activities; however, they also intend to move the state closer to TB elimination by more widely promoting targeted testing, contact follow-up, and short course LTBI treatment regimens. The TB program staff will continue regular conference calls with PHD staff every other month. The TB Controller and the Public Health Physician will review diagnosis and treatment for suspects and active cases. The Epidemiology Section Manager will help coordinate case reporting and follow-up activities with the TB Controller. Subgrants with the PHDs to carry out day-to-day case management and treatment activities (including contact investigations) will continue to be managed by the Epidemiology Section Manager. The TB program staff also aim to establish a TB elimination advisory group to assist in the development of an elimination plan. The Epidemiology Section Manager will be the point person for coordinating and carrying out surveillance for TB cases, case reporting, and program evaluation activities. These activities will include data management, regular review, and evaluation of reporting of TB suspects and cases, outbreak surveillance, reporting aggregate contact investigation data, and ensuring timely and complete reporting of domestic evaluations of immigrants and refugees. During the new project period, the Idaho TB program staff intend to carry out at least 4 program evaluation projects based on progress toward meeting NTIP performance metrics. The Human Resource Development section of this application focuses on how Idaho TB program staff intend to maintain a trained TB workforce in a low incidence state with limited resources. The main objective is to utilize existing educational opportunities for in-state TB training (e.g., state-wide epidemiology conferences, TB case review conference calls) and, as resources allow, send PHD staff to out-of-state TB-specific trainings. The Laboratory Strengthening section of this application focuses on improving current TB testing practices. The IBL is the only reference laboratory in Idaho with the capacity to perform the full spectrum of TB testing services. IBL provides high quality specimen processing, rapid testing, mycobacterial culture and identification, and first-line drug susceptibility testing routinely. For the past 5 years, 100% of isolates from Idaho have been submitted to reference laboratories for genotyping. The laboratory consistently achieves scores of 100% on mycobacterial proficiency surveys and has been enrolled in the CDC Model Performance Evaluation Program for drug susceptibility testing since 2009. During the new project period IBL will continue to provide core TB testing services and look for innovative ways to improve turnaround times for reporting results to ordering providers.
