Project Fast Track Fast tracking evidence based scalable responses to achieve 95-95-95 targets in India through Technical Assistance to NACO Project Summary PEPFAR cluster states contribute to 1/3rd of HIV infections in the Country. With the overall objective to improve effectiveness and efficiency of national HIV program to achieve epidemic control, VHS will partner with NACO and NTEP in five states (18 prioritized districts; ROP 2019), to fast-track response towards epidemic control. It will build on achievements of NACO and PEPFAR partners to augment case findings and linking to care. Keeping in mind the NSP targets for HIV and TB, this proposal looks at strategies for rapid scale-up of tailored prevention and care activities among KP (MSM, FSW, TG, PWID and Prisoners) and vulnerable population (adolescents, youth, TB patients) across the care continuum. Prevention efforts will focus on case finding among emerging KP (e.g. virtual platform users) and new KP sites (e.g. spas, massage parlors). Apart from providing need based TA to NACO on key areas to strengthen program implementation, as per TI revamp strategy, this proposal will refine, innovate and scale-up prevention to care strategies like social marketing for self-test kits through kiosk machine, prioritised index testing for intensified case detection and C34QL a single window comprehensive care model for same day ART enrolment, TB screening and PrEP for high risk seronegative KPs. To provide focused care to TGs, capitalising on its experience, VHS proposes to scale-up gender clinics (TG) to other districts. Transit strategies for prisoners to ensure uninterrupted prevention and treatment services (IDUs) have been enlisted. VHS will further support NACO on HIV-TB activities within prison settings. New rates of infection among youth in North Eastern states has necessitated considering activities that can be incorporated into local culture (youth groups, music choir). Models of differentiated prevention for FSW, MSM and TG were implemented in one state which is proposed to be scale-up for PWIDs in the North East. Elimination of HIV is dependent on ensuring adherence to ART and viral suppression. VHS will provide Technical Assistance to NACO on scaling up care cascade, including TB prevention/TPT to high KPs and transitioning PLHIVs on treatment to TLD. To ensure adherence and retention on ART, apart from supporting NACO on DSDM services, this proposal proposes piloting nurse and community led DSDM models. VHS has implemented Jindagi an app-based tool for ART adherence and retention for Mumbai DACS. It is proposed to scale-up this app to other cluster districts. This tool will be enhanced, to provide two-way communication, helping case-based surveillance and prevent LFU. An epidemiological tool to identify clusters that require focused efforts to facilitate case detection is being proposed. Strengthening health and community engagement for scale-up from prevention through treatment cascade has been built into every stage of this proposal. This will ensure quality care is provided and targets are met. Given that the activities will be implemented through existing health systems, building capacity and mentoring health care staff will ensure sustainability of project activities beyond the project period. This proposal seeks to create institutional memory of PEPFAR activities by creating a national repository. Cross learning and sharing between partners and stakeholders and from within the project have been incorporated into the proposal. The detailed proposal presented covers all activities mentioned in the NOFO. VHS has received US Government grants for 25 years and has successfully demonstrated capacity to achieve targets for the grants awarded.
