Background: The consortium comprising of The AIDS Support Organization (TASO) as the prime; University Research Co.,LLC (URC) and Uganda Fisheries and Fish Conservation Association (UFFCA) is presenting this project proposal to aggressively accelerate epidemic control through attainment of 90-90-90 UNAIDS targets by 2020 and reinforcing health systems in Soroti Region in FY17, FoA CDC-RFA-GH17-1710, US $9,775,895. In FY17, The consortium shall implement the following objectives: 1: To provide HTC services to 30,400 individuals so as to increase the proportion of PLHIV identified in Soroti Region from 63% to 90%; 2: To expedite provision of antiretroviral therapy (ART) to 37,315 PLHIV current on ART and 4,326 new naive diagnosed with HIV infection from 77% to 90% in Soroti Region ; 3: To accelerate access to eMTCT services among 565 HIV positive pregnant and lactating mothers from 75% to 95% in Soroti Region; 4: To increase the proportion of 41,641 PLHIV on ART who attain Viral Load (VL) suppression from 70% to 90% in Soroti region ;5: To rapidly accelerate the coverage of high impact evidence-based combination HIV prevention interventions among Key/Priority Populations from 80% to 90% for 2,022 sex workers, 40% to 90% for 17 MSM, 471 police personnel, 982 sero-discordant couples;, 25% to 90% for 16,830 fisher folks & 565 Army personnel from in Soroti Region; 6: To provide 8,403 eligible OVCs and their families with Healthy, Safe, Stable, Schooled and Case management services to support the continuum of response (COR) toward 90-90-90 and graduate families out of vulnerability in Soroti Region; 7: To strengthen leadership, stewardship and governance systems in 219 public health facilities for sustained epidemic control in Soroti Region; 8: To strengthen the strategic information systems (OpenMRS, DHIS2, DATIM, OVCMIS and HIBRID) in 219 public health facilities for sustained epidemic control. Gap in the National HIV/AIDS Reponses: Uganda is categorized as a low income country, with a GDP per capita of US$696.4 and per capita health expenditure of US$59 (The World Bank, 2015). Uganda is still experiencing a generalized epidemic with adult HIV prevalence increasing from 6.4% to 7.3 percent (Ministry of Health, 2011). Soroti region has a total population of 1,819,708 (5.3% of the country) with Females accounting for the highest percentage of 51% (92,336) and Males 49% (895,372). The region has an estimated HIV prevalence of 5.3% (UAIS 2011). Soroti region has 90,543 PLHIV adults and children, of which 36,842 (40.69%) are currently receiving antiretroviral therapy (ART), and 42.89% of PLHIV receiving at least a clinical assessment or CD4 or VL (COP2015). This indicates a critical gap of 59.3% for ART coverage, and 57.11% VL assessment coverage that is expected to be addressed by the consortium. Implementation Strategy: TASO Strategy shall comprise of: 1. Clustering Model: TASO used the number of health facilities (DHIS2) and PEPFAR scale-up categorization to cluster Soroti Region into 4. Cluster 1 (Scale-up districts Soroti and Katakwi), Cluster 2 (Kaberamaido and Amuria), Cluster 3 (Serere and Ngora) and Cluster 4 (Bukedea and Kumi). 2. Community Based Programing (CBP), 3. Direct Service Delivery (DSD), TASO shall provide DSD at TASO Soroti. 4. Technical Assistance (TA), 5. Sub Granting, TASO shall channel funds to other implementing partners (URC, UFFCA) and CSO/CBOs 6. Coordination and Collaboration, 7. Evidence-based HIV Combination Prevention Model (HTC, PMTCT, VMMC, PHDP and RH/FP, TB/HIV, and Comprehensive HIV Prevention and Care for KP/PP. 8. Peer-to-Peer Model, using the snowballing approach to extend care and support to sexual minorities. 9. Continuous Quality Improvement (CQI). This strategy shall be integrated at all levels of project implementation. 10. Layering of Services for OVCs and AGYWs.
