The purpose of this Cooperative Agreement is to request continued federal financial and direct assistance for 12 months (July 1, 2019 - June 30, 2020), to support efforts to plan, develop, and maintain Idaho's public health infrastructure. The funds will assure high immunization coverage levels and low incidence of vaccine-preventable diseases. Idaho Immunization Program Program Abstract The purpose of this Cooperative Agreement is to request continued federal financial and direct assistance throughout the budget period to support efforts to plan, develop, and maintain Idaho’s public health infrastructure to assure high immunization coverage levels and low incidence of vaccine-preventable diseases. The state of Idaho ranks 13th in total area in the United States and 11th in total dry land area. It is 490 miles in length from north to south and at its widest point, 305 miles east and west. Idaho has 44 counties and a land area of 84,033 square miles with agriculture, forestry, manufacturing, and tourism being the primary industries. The bulk of Idaho's landmass is uninhabited and inhabitable due to the natural deterrents of desert, volcanic wastelands and inaccessible mountainous terrain. Eighty percent (80%) of Idaho's land is either range or forest, and 70% is publicly owned. The state has seven major population centers. Southern cities follow the curve of the Snake River plain and are surrounded by irrigated farmland and high desert. Lewiston, in north central Idaho, is centered in rolling wheat and lentil fields, and deep river canyons. In north Idaho, Coeur d'Alene is located on a large forested mountain lake and is a major tourist destination. Much of the state's central interior is mountain wilderness and national forest. The isolation of many Idaho communities makes it difficult and more expensive to provide health services. There are twelve program areas, as defined by the Centers for Disease Control and Prevention’s National Center for Immunization and Respiratory Disease, that activities are geared towards; 1. Program Management and Sustainability, 2. Program Effectiveness, 3. Vaccine Access and Management, 4. Immunization Information Systems and Technology, 5. Communication, Partnerships, and Education, 6. Provider Quality Improvement, 7. Surveillance and Outbreak Response, 8. Perinatal Hepatitis B Prevention, 9. Pandemic Influenza Preparedness, 10. School Vaccination Coverage, 11. Adolescent Immunization, and 12. Adult Immunization. In accordance with the 2019 Immunization and Vaccines for Children Program Funding Opportunity Announcement CDC-RFA-IP19-1901 by the Centers for Disease Control and Prevention’s (CDC) Procurement and Grants Office, this cooperative agreement application consists of required Public Health Service forms, a narrative section that addresses objectives and required and awardee-defined activities for the twelve program areas and a section detailing the budget justification. This grant is managed by Kathryn Turner, Ph.D , Bureau Chief, Bureau of Communicable Disease Prevention. The grant supports immunization program staff located at the Idaho Immunization Program. The grant-supported staff at the Idaho Immunization Program is responsible for administration of all grant activities.
